Request Your Health Care Records

You may request a copy of your health care records by contacting Next Chapter Counseling of Texas directly. Please submit a written request by email, mail, or through our contact form, and we will respond according to Texas law and applicable privacy regulations.

Contact the Licensing Council

If you have questions about professional licensing or regulation, you may contact the Texas Behavioral Health Executive Council through the Contact Us page on their website.

File a Consumer Complaint

If you believe your consumer rights have been violated, you may file a complaint with the Texas Office of the Attorney General’s Consumer Protection Division through the Consumer Protection webpage on their website.

At Next Chapter Counseling, you have the right to access your health care information and to contact state agencies with questions or concerns about your care.

An individual who wishes to file a complaint against a Licensed Professional Counselor and/or Associate may go to https://bhec.texas.gov/discipline-and-complaints/ to download the approved complaint form to mail. Alternatively, you may call or write and request the complaint form be mailed to you.
